Enter the Lot Number shown on your property tax bill. This is a three digit number sometimes followed by a letter (example: 321 or 654B). 

This is combined Block and Lot (example: 1023 654B). 

Enter the first name of the property owner. 

Enter the last name of the property owner. 

Enter the name of the property owning entity (for example: Property Management LLC) 

Enter the street address of the taxable property (example: 123 Sample Street or 567 Anyplace Blvd). 

This is an additional or secondary street address of the taxable property. 

Enter the unit or suite number of the taxable property (example: #3 or Unit 50 or Suite C). 

Enter "San Francisco". 

Enter "CA". 

Enter the Zip Code of the taxable property (example: 94102 or 94103-0001). 

This is the property address that will appear on your submitted application. It's the combined property street address, unit, city, state and zip code.

Enter the street address where you receive your tax bills (example: 123 Sample Street or 567 Anyplace Blvd). 

This is an additional or secondary street address where you receive your tax bills.
